Electronic visit verification (EVV) is a digital system that captures home care visit information. As per the 21st Century Cures Act, its use is federally mandated across the United States.

EVV automatically tracks six key data points for each appointment:

  • The type of service performed.
  • The name of the individual receiving the service.
  • The date and location of service delivery.
  • The caregiver’s clock-in and clock-out times.

When You Need an EVV Correction

1. Late or Missed Punches

EVV enables caregivers to clock in and out with a single tap. However, they may still forget to use this function promptly, such as due to distractions or overwhelm.

2. Wrong Date or Time

These mistakes are particularly common when a caregiver does not log their visit in real time. If the recorded time doesn’t match the actual hours worked, a correction is mandatory.

3. Incorrect Service Code, Client, or Caregiver

EVV service codes are highly specific data points that help document which service was provided for billing and compliance. Entering “personal care” instead of “respite,” for example, can lead to major billing troubles. A correction is also needed if a caregiver selects the wrong client from a drop-down menu or if a substitute caregiver logs in under the primary’s ID.

4. Technical Failure

Sometimes a device’s GPS can’t confirm a caregiver’s coordinates, even though they’re exactly where they need to be. In other cases, data might fail to upload due to poor cell service. App malfunctions and dead device batteries also throw a wrench in the EVV process.

5. Supervisor Adjustments

A supervisor may spot inconsistent data during a weekly audit, or documentation may not align with the care plan. In these instances, the supervisor may request a correction form to double-check the information before the billing department receives it.

How to Use Our EVV Correction Form Template

For Caregivers

The correction process begins with frontline staff. Using the EVV app, caregivers must pinpoint the exact error that requires an amendment.

Was it a forgotten clock-out, mislabeled service, or something else entirely?

Once a caregiver identifies the issue, they must then enter the correction. On the form, there are two columns: “Original Information” and “Corrected Information.”

Be sure to update all affected fields, including the date of service, service code, start and end times, location, and services provided. Fill in all of the necessary data, including the correction and the associated reason code, with precision. In the “Notes” section, briefly explain the reason for the correction and any relevant context.

Since an EVV correction is a compliance record, the caregiver must sign off on the information provided.

For Administrators

Administrators must approve the proposed changes. The supervisor must sign and date the form to formally document approval.

Before hitting “Approve,” verify that the corrected hours (if applicable) don’t exceed the client’s weekly authorizations, as that can trigger a billing rejection later on.

Next, double-check the caregiver’s explanation to make sure everything checks out. If a caregiver frequently cites technological issues as the reason for their corrected punch times, it may indicate a training issue or device problem rather than a glitch.

Once the form is validated, caregivers should manually adjust the visit information within the EVV software. A scan or digital copy of the correction should be attached to the associated record to create a clear audit trail.

To wrap everything up, caregivers must submit the documentation within the agency’s or state’s required correction timeframe. The submitted and approved dates should clearly reflect compliance with those deadlines. If it’s late, ensure you’ve documented the cause for the delay.
